Nursing Coordinator/ Intake Nurse

Preferred Health Care is looking for a Registered Nurse to join our team. This person is responsible for doing client intakes and quarterly reviews and care plans and documentation according to the State of Massachusetts regulations.

The ideal candidate will display compassion, education, proficiency and experience while assisting the patient. This critical role assists in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient.

Responsibilities:

Coordinate intakes with new clients
Conduct administrative duties that go along with the intake process.
Conduct quarterly reviews with clients
Annual skills for the CNA's and HHA's

Requirements:

Bachelor's degree and a license to practice nursing in the state required
A minimum of six months of recent medical experience
Must be able to stand or sit for long periods of time

This position is for a Registered Nurse who has a license in Massachusetts and will be in various locations such as Fall River, New Bedford, Dartmouth, Fairhaven, Somerset, Swansea, Seekonk, Rehoboth, Dighton
